Team-Based Care
Implementing an integrated system of primary and community care built on team-based care is a key priority in British Columbia. This person- and family-centered model is designed to provide comprehensive shared care through a strong, interdisciplinary team of health care providers. Moving to this integrated system involves a new way of practicing for many health care providers.
What is Team-Based Care?
“Multiple health care providers from different professional backgrounds work together and with patients/clients, families, caregivers and communities to deliver comprehensive health services across care settings. Effective teamwork is a critical enabler of safe, high quality care and supports a patient’s ongoing relationship with their primary care provider (a family physician or nurse practitioner).”1
The journey to transition to quality, team-based care with patients and providers as equal partners means sharing and accessing resources, events and opportunities to support everyone along the way.

What Guides This Work?
The Team-Based Care Advisory Group guides work across the province to accelerate the transition to quality, team-based care by providing systems of support for local action and change with patient and providers as equal partners. Membership of the Team-Based Care Advisory Group is from across the province and works to help support “quality, person- and family-centred and collaborative care that provides the right care, at the right time and in the right place, in a sustainable system.” Learn more about their shared purpose.
